Cancellation Policy
If you are unable to keep your appointment, kindly give us 24 hours notice. Otherwise, we reserve the right to charge for time reserved. We work on a timely schedule. Any patient who is more than 15 minutes late will have to be rescheduled.
Broken Appointments
Our office operates on a very high hourly overhead cost basis. We schedule your time with us just for you. When you do not show up, many people in our office are left with nothing to do. We cannot possibly find a patient to fill in your time slot in such a short period of time. As a result, we must absorb these overhead costs of lost income. Therefore, our policy is that if a patient has two broken appointments in a year, they will receive a letter stating that if one more broken appointment occurs, we will not schedule any future appointments. If a family appointment is broken, we will only be able to schedule an appointment for one family member at a time. Also, if an appointment that is scheduled for more than two hours is broken, that will constitute as two broken appointments for that patient.
Obtaining Dental Records
If you have been under the care of another dentist in the last five years, please contact them to find out the date of your last cleaning and if any x-rays were taken of your teeth. If x-rays have been taken, please have the dental office send a copy of these to us. A copy of the form "Release of Records" can be downloaded under "Patient Forms".
Financial Policy
Our office accepts all insurance and will gladly file your insurance claims for you. We are participating providers with United Concordia, Delta Dental, Dentemax, Guardian, Connection Dental, Anthem BlueCross BlueShield, Mt. State BlueCross BlueShield, 4 Most, and WellCare.
If you have dental insurance, make sure you bring your ID card so that we can verify coverage and get a breakdown of your benefits. We also ask that our patients who are covered under dental insurance pay their estimated portion and any deductibles or co-payments that have not been met at the time of services are rendered. Please try to arrive approximately 15 minutes before your appointment so we may verify your insurance and seat you promptly for your appointment.
Private-pay patients receive a discount when paying in full on the day of service. We offer financing through Capitol One and Care Credit. Call our office for detailed information or to apply. It only takes minutes to be approved. Unfortunately, we cannot accept payment plans for private pay patients.
We are participating providers with Medicaid, WV Chips, UniCare, Carelink, and Pre-Employment services. If you have a medical card with UniCare and Carelink and are over 21 years of age, you must have your UniCare or Carelink card also. If you are covered under Pre-Employment, you must have the pink copy of your enrollment form. Patients who arrive without their insurance cards will have to be rescheduled.
